What Patients Need To Bring On Their First Visit
Going to a new doctor means filling out forms. It’s an expected part of the process. Working with a chiropractic office, though, is not quite the same as going to a medical office. There are certain things in your medical history that will be incredibly helpful for us to know. Learning this information can be vital in properly understanding your problem and making the best care plan for you specifically. With that in mind, here are some things that we recommend you bring or know, so our doctors can provide you with the best possible care:
- Contact information for your other doctors
- Medical records (including diagnostic and imaging tests, such as x-rays and MRIs)
- Insurance information, if available
- Information on any previous injuries (e.g., automobile, sports, or workers’ compensation accidents)
- Information on any previous circulatory incidents (e.g., a stroke or aneurysm)
- Information about any heart conditions (e.g. high blood pressure)
- Information on bone problems and disorders (e.g., fractures or osteoporosis)
- Information on any previous disc injuries (e.g., bulging discs, protrusion, or herniation)
- Information on joint disorders (e.g., arthritis)
- Details on any episodes of dizziness or blurred vision that you have suffered
- Details on past or present infections
- Details on past or present autoimmune diseases
- Details on all other health issues
Want to save yourself some time at your initial appointment? Fill out our New Patient Form in advance of your first visit. It includes our health questionnaire, so you have plenty to time to think about your answers and seek out any associated records or information you think may be valuable. You will need to download the form, print it out, and fill in the required information. After you have completed the form, you are welcome to fax it to our offices – or simply bring it with you to your appointment.
AdobeReader® is required to download the form. If AdobeReader® is not installed on your computer, click here to download.
What Will Happen At Your First Appointment
Meeting with a new doctor for the first time can be stressful. Especially if you are not sure exactly what to expect. To alleviate new patient fears, this section breaks down each part of your visit in detail, so you know what will happen from the moment you walk into our doors to the second you leave.
Your initial visit to our Cumming office will be broken down into six basic parts:
As soon as you enter Wellness For Life, our receptionist will welcome you as a member of our family. Once the receptionist learns that this is your first visit in our office, you will be asked if you have completed the New Patient form(s) mentioned in the above section and whether they were emailed in or with you.
If you have not completed them, please come in 10 minutes earlier so you have time to do so in the office.
We want to make sure both of us are well informed. We will have you fill out a symptom survey and read a small pamphlet about us and what will happen in our office.
The first part of your medical appointment in our office will consist of a physical exam where we will get some of your vitals: Heart Rate, Blood Pressure, Blood Oxygen Saturation, Weight, and Body Fat %. We will also perform a Heart rate variability Test, which is a relaxing non-invasive measurement of the overall ability of your Autonomic Nervous System's to adjust and adapt to your environment.
The second part of your medical appointment will be with your doctor and will consist of a consultation. The doctor will sit down with you and discuss your concerns and goals regarding your health, whether you are a chiropractic or nutritional case, and how we may be able to help.
Listening to you describe your issues, expectations, goals, and overall lifestyle, and helping you understand what we do, will help the doctor determine how our health care can best meet your goals and which treatment options are most likely to work for you.
The doctor will next perform a complete examination depending on what your visit is for. This will include all or some of the following test: Manual muscle testing, neurological reflexes, orthopedic tests, palpation, and evaluation of autonomic nervous system reflexes. It goes without saying, of course, that nothing in our office will be done without your consent.
Once the examination is complete, the doctor will go over the results to let you know what he found, what it means, and whether or not your condition requires care with other providers. Chiropractic, nutrition, biofeedback, massage and other things we offer in our office can help with a great many things, but some issues are outside of our scope. If that is the case, we can recommend providers that we work with and trust.
At this point, your doctor will also be happy to answer any questions you might have. Common questions that many patients have at this point include:
- Can you help me?
- How often do you need to come in?
- How long will it take?
- What will your treatment cost?
Finally, your doctor will tell you what kind of treatment plan they recommend for you.
The final part of your visit. Before you leave, the doctor may make additional recommendations on procedures or exercises for you to perform between visits to optimize your results.
For example, if you are in pain at the time of your visit, your Wellness Program may include:
- Instructions on applying ice or heat
- Certain activities or positions to avoid
- At home exercises and/or stretches
Our wellness team is also happy to work with you to help you create healthy habits and routines that fit with your lifestyle. The purpose of our wellness program is simple: an overall improved quality of life. That means not only helping you achieve proper mechanics and nervous system communication, but also help you learn what nutrition really is, what sort of exercise is most beneficial, and techniques to reduce stress and maintain a positive mental state.
Options For Cumming Residents To Pay For Our Wellness Care
At Wellness For Life, we believe that everyone should be able to experience the benefits of high-quality chiropractic care – no matter what your budget is. Because of this, we have carefully planned our pricing structure and accept a wide variety of payment options, including flexible payment programs when necessary.
Our office accepts most insurance plans, and also accepts select auto accident, workers’ compensation, and personal injury cases.
Since specific plans and level of coverage vary widely, the best way to determine whether or not your plan will cover care in our office is to give us a call. Our experienced staff members are always happy to verify your coverage and explain your benefits before you undergo any treatment.
We understand that many people simply do not have insurance. Or you may have insurance, but it does not cover receiving chiropractic care. If this is your situation, there are still options.
Many of our patients pay directly for care, and are surprised to discover how cost-effective and affordable chiropractic treatments are when compared to other types of medical care.
There are a number of ways that you can pay directly. We may be able to create a customized plan specific to you and your family. As mentioned above, flexible payment options may be able to be arranged.
We accept cash, credit, debit, and checks.
Bottom line: do not neglect your health because you feel like you can’t afford the cost – either in money or time. Whatever your situation, please contact us. We will try to help!
